چکیده

We would like to congratulate Fruchter et al 1 for their ingenious, effective, and minimally invasive way of treating the very challenging condition of bronchopleural fi stulas as reported in a recent issue of CHEST (March 2011). However, because we were cited in their work, we must make a correction. The misunderstanding is that our fi rst publication was a case report of our fi rst patient (53-year-old man with a right upper lobectomy) using this method. 2 Later, Scordamaglio et al 3 published a review of our experience endoscopically treating airway fi stulas (two bronchopleural and one tracheoesophageal), which included this same patient. Since then, fi ve more fi stulas in four other patients have been treated, making a total of seven fi stulas treated in six patients (one patient with two fi stulas), with fi ve of them already closed. Although the technique used is similar to that in Fruchter et al, 1

Elevated pulmonary artery pressure is a risk factor for primary graft dysfunction following lung transplantation for idiopathic pulmonary fibrosis.

BACKGROUND Idiopathic pulmonary fibrosis (IPF) is often associated with elevations in pulmonary artery pressures. Although primary pulmonary arterial hypertension (PAH) has been associated with primary graft dysfunction (PGD), the role of secondary PAH in mediating PGD risk in patients with IPF is incompletely understood. The purpose of this study was to evaluate the relationship between mean p...

Cardiac diastolic function and hypercapnic ventilatory responses in central sleep apnoea.

Hyperventilation is the key factor contributing to the development of idiopathic nonhypercapnic central sleep apnoea (ICSA), where left ventricular systolic function is normal. ICSA is reported to occur in 20% of patients with left ventricular diastolic dysfunction, in whom elevated pulmonary vascular pressures and resultant increased pulmonary vagal afferent traffic may contribute to hypervent...

Novel 24-h ovine model of brain death to study the profile of the endothelin axis during cardiopulmonary injury

BACKGROUND Upregulation of the endothelin axis has been observed in pulmonary tissue after brain death, contributing to primary graft dysfunction and ischaemia reperfusion injury. The current study aimed to develop a novel, 24-h, clinically relevant, ovine model of brain death to investigate the profile of the endothelin axis during brain death-associated cardiopulmonary injury. We hypothesised...

Preoperative Cardiac Variables of Diastolic Dysfunction and Clinical Outcomes in Lung Transplant Recipients

Background. Orthotopic lung transplantation is now widely performed in patients with advanced lung disease. Patients with moderate or severe ventricular systolic dysfunction are typically excluded from lung transplantation; however, there is a paucity of data regarding the prognostic significance of abnormal left ventricular diastolic function and elevated pretransplant pulmonary pressures. Met...

THE EFFECTS OF CAPTOPRIL ON PULMONARY AND SYS TEMIC ARTERIAL PRESSURES IN HIGHALTITUDE PULMONARY HYPERTENSION

The purpose of this investigation was to assess the effect of captopril on both systemic (P.a) and pulmonary arterial pressures (PPA) in patients with high-altitude pulmonary hypertension (HAPH). Seventeen patients (mean age 44±6.8 years) with HAPH and mild to moderate systemic arterial hypertension were included in the study. All patients underwent right heart catheterization with measurem...

Pulmonary Hypertension in Elderly Patients with Diastolic Dysfunction and Preserved Ejection Fraction

PURPOSE Patients with diastolic dysfunction may have a disproportionate degree of elevation in pulmonary pressure, particularly in the elderly. Higher pulmonary vascular resistance in the elderly patients with heart failure but preserved ejection fraction suggests that beyond the post-capillary contribution of pulmonary venous congestion, a pre-capillary component of pulmonary arterial hyperten...
